Has the equipment been working fine since it was supplied? Was the equipment Dismantled or moved at any time after it was installed?
Exactly what is the failure type - please describe in detail what is working and what isn’t
What has been tried to rectify the issue? What was the result?
Any other relevant detail eg. was there a power cut just before the failure?
N.B. If possible provide any graphical materials that show the condition of the machine.
